This Privacy Policy explains how Personalized Life Style Nutrition DBA, doing business as Trugenex (“Trugenex,” “we,” “us,” or “our”), collects, uses, discloses, retains, and protects information through:

* The Site’s general inquiry form;

* General telephone calls and voicemail; and

* Related email communications.

This Privacy Policy applies only to information processed by Trugenex through these channels.

1. Information you provide through the form

The Site’s general inquiry form may collect:

Where applicable law treats identifiable health-related message content as consumer health data:

* It is not provided to data brokers;

Submitting an inquiry does not automatically enroll you in a recurring newsletter or promotional email campaign.

We may send communications necessary to respond to your inquiry. Separate permission will be requested before sending recurring marketing communications when consent is required.

6. How information is disclosed

Trugenex does not disclose personal information except as described in this Policy.

The provider you select

The information submitted through the form may be routed to the provider you selected and to Trugenex personnel who reasonably need the information to administer and respond to the inquiry.

Submitting the form does not guarantee that the selected provider will personally respond.

Trugenex personnel

Authorized employees and contractors may access information when reasonably necessary to:

* Route inquiries;

* Respond to visitors;

* Address privacy requests;

* Protect information and systems; or

* Comply with law.

Service providers

7. No sale or targeted advertising

Trugenex does not:

* Provide form submissions to data brokers;

* Use form content for targeted advertising;

* Share personal information for cross-context behavioral advertising;

* Build advertising profiles from nutrition or wellness inquiries; or

* Allow advertising networks to use contact-form information.

Because Trugenex does not engage in these practices, there is no separate sale or targeted-advertising opt-out process currently required for Trugenex’s ordinary Site operations.

A visitor may still submit a privacy request using the procedure below.

8. Cookies and similar technologies

The Site may use cookies or similar technologies to:

* Operate webpages;

* Maintain security;

* Prevent spam;

* remember basic preferences;

* Diagnose technical errors; and

* Understand aggregate Site traffic.

Limited analytics tools may receive technical information such as Internet Protocol address, device type, browser type, requested page, referring page, and time of access.

Trugenex does not authorize analytics providers to receive the contents of contact-form messages for advertising purposes.

You may be able to control cookies through your browser settings. Blocking all cookies may prevent some Site features from working properly.

9. Browser privacy signals and cross-site tracking

Some browsers provide “Do Not Track” signals. Because there is not a universally adopted technical or legal standard governing every Do Not Track signal, the Site may not respond to those signals in a uniform manner.

Trugenex does not use contact-form information for cross-site behavioral advertising.

Trugenex will recognize a legally required browser-based opt-out preference signal when such a signal applies to Trugenex’s practices and applicable law requires a response.

Website hosting, security, analytics, video, map, or other third-party services may receive limited technical information when their services are used. Their processing may be governed by their own privacy policies.

The new Site should not include advertising pixels, retargeting tools, or session-replay software unless this Policy is reviewed and revised before those technologies are activated.

10. Email communications

When you provide an email address, Trugenex and the provider you selected may use it to respond to your inquiry and provide related administrative information.

Ordinary email carries security risks. Do not send unnecessary sensitive, financial, identification, or detailed health information by email.

Trugenex does not guarantee that an email will be received, reviewed, delivered, or responded to within a particular period.

A person may ask Trugenex not to send further nonessential email. Trugenex may still retain and send communications reasonably necessary to complete a pending inquiry, comply with law, or maintain appropriate records.

11. Retention

Trugenex retains information only for as long as reasonably necessary for the purposes described in this Policy.

Unless a longer period is required for legal, security, or dispute-related purposes, Trugenex’s ordinary retention periods are:

* Contact-form inquiries and related email correspondence: up to 24 months after the most recent substantive interaction;

* Routine telephone and voicemail records: up to 24 months after the most recent substantive interaction;

* Routine technical, security, analytics, and diagnostic logs: up to 12 months;

* Spam, fraudulent, or malicious submissions: for as long as reasonably necessary to prevent abuse;

* Privacy requests and records showing how they were resolved: for the period reasonably necessary to demonstrate compliance with applicable law; and

* Deidentified or aggregate information: for as long as it remains nonidentifiable and useful for a legitimate business purpose.

Information may be retained longer when reasonably necessary to:

* Comply with law;

* Preserve evidence;

* Establish, exercise, or defend legal rights;

* Address fraud or security incidents;

* Enforce an agreement; or

* Complete an ongoing service relationship.

Deletion from active systems may not result in immediate removal from every backup. Backup information will be deleted or overwritten through ordinary backup-management cycles, subject to applicable legal requirements.

12. Information security

Trugenex uses reasonable administrative, technical, and organizational measures intended to protect information against:

* Unauthorized access;

* Loss;

* Misuse;

* Improper alteration;

* Improper disclosure; and

* Destruction.

Measures may include:

* Limiting access to people who reasonably need the information;

* Using reputable hosting and communication providers;

* Maintaining account and access controls;

* Monitoring for spam or malicious activity; and

* Maintaining procedures for responding to security incidents.

No website, email system, telephone system, or storage method can be guaranteed to be completely secure.

Visitors can reduce risk by avoiding unnecessary sensitive information in the public form, email, or voicemail.

1. Use the Site’s contact form and begin the message with “PRIVACY REQUEST”; or

2. Call (405) 245-9475 and state that you are making a privacy request.

Please describe:

* The right you wish to exercise;

* The information or interaction involved;

* The email address associated with the inquiry; and

* Any information reasonably necessary to locate the relevant record.

Do not include a Social Security number, government identification image, financial-account information, or password in a privacy request.

Verification

Trugenex may take reasonable steps to verify that the request comes from the person to whom the information relates or from an authorized representative.

Verification may involve confirming access to the email address or telephone number previously provided.

Trugenex will request only the information reasonably necessary to verify the request.

Authorized agents

An authorized agent may submit a request when permitted by applicable law.

Trugenex may request evidence that:

* The agent is authorized to act for the individual; and

* The identity of the individual has been reasonably verified.

Responding to requests

Trugenex will respond within the period required by applicable law.

A request may be denied or limited when an exception applies, including when information must be retained for:

* Protection of another person’s rights;

* Completion of a requested transaction;

* Establishment or defense of legal claims; or

* Another legally permitted purpose.

When required by law, Trugenex will explain the reason for a denial.

Describe the original request and why you believe the decision should be reconsidered.

14. Children’s information

The Site is intended for a general adult audience and is not directed to children under 13.

Trugenex does not knowingly request or collect personal information directly from a child under 13 through the public form.

A parent or legal guardian may submit a general inquiry concerning services for a minor.

When Trugenex learns that a child under 13 submitted personal information directly without appropriate parental involvement, Trugenex will take reasonable steps to delete it unless retention is required to protect the child or comply with law.

A person under 18 should use the Site only with the involvement of a parent or legal guardian.

15. Third-party websites and embedded services

The Site may contain links to or integrations from third-party services, such as:

Information may be stored or processed in the United States. Privacy laws in the United States may differ from the laws of another country.

The Site is not intentionally directed to a jurisdiction in which operating the Site or processing the information described in this Policy would be unlawful.

Trugenex may update this Policy to reflect changes in:
